ABSTRACT

Objective:To establish an animal model of acute systemic cold injury in mice.Methods:There were 98 C57BL/6 mice, half male and half female, with body weight of 22-27 g and age of 10 weeks. The mice were randomly divided into 7 groups ( n=14) according to the changes of anal temperature in cold environment, namely, group A (38.5 ± 1) ℃, group B (35 ± 1) ℃, group C (30 ± 1) ℃, group D (25 ± 1) ℃, group E (20 ± 1) ℃, group F (15 ± 1) ℃, and group G (10 ± 1) ℃, among which, group A was the blank control group, and the rest groups were the experimental group. The mice in the blank control group were placed in the normal environment (20 ± 5) ℃, and the mice in the experimental group were placed in the low temperature artificial climate box at - 20℃. The anal temperature of the mice was measured intermittently (as the core temperature), and the time required for the core temperature of the mice to drop to groups B, C, D, E, F and G was recorded. The righting reflex was used to evaluate the consciousness state, the action ability and the general state of each organ of mice were observed, and the blood routine and HE staining of each organ were detected. Results:The lower the core temperature of the experimental group, the longer the time required. The consciousness state, action ability, general state of organs, blood routine, and HE staining of organs in groups B, C, and D were basically the same as those in group A, and there was no acute systemic cold injury. Therefore, the blood routine, general observation of organs, and HE staining of organs in groups B, C, and D were no longer displayed compared with those in group A. Compared with group A, mice in group E began to suffer from disturbance of consciousness and action ability. With the decrease of core body temperature, the damage was aggravated, and mice in group G died. Compared with group A, the indices of blood routine test (WBC, RBC, HGB, PLT) of mice in group E began to decrease, and the univariate variance calculation showed that only WBC changes had statistical significance ( P<0.05). Compared with groups A and E, the indices of blood routine test (WBC, RBC, HGB, PLT) of mice in group F were further reduced, and the changes of each index in univariate variance calculation were statistically significant ( P<0.05). The general observation results showed that compared with group A, the lung, liver and spleen surfaces of mice in group E began to darken, and compared with groups A and E, the lung, liver, spleen, kidney and heart of mice in group F were further deepened and darkened, with irregular edges. HE staining results of various organs showed that compared with group A, the mice in group E began to have partial alveolar structure destruction and a small amount of inflammatory cell infiltration, the central vein of the liver was slightly congested, and the red and white pulp of the spleen were indistinct. Compared with groups A and E, the pathological structure damage of the lung, liver, spleen, kidney, heart and brain tissues of the mice in group F was further aggravated. Conclusions:Detection of consciousness state, action ability, general state of organs, blood routine and HE staining indices of organs in mice under low temperature can simulate the progress of clinical acute cold injury, and the animal model of acute systemic cold injury was successfully prepared.

ABSTRACT

Huangtu Decoction, first recorded in Essentials from the Golden Cabinet(Jin Kui Yao Lue) from ZHANG Zhong-jing in Han dynasty, is used to treat distal bleeding. It is mainly treated for the syndrome of failing to control blood with spleen-yang deficiency. The connotation of distal bleeding is more extensive, including not only upper gastrointestinal bleeding in the traditional sense such as peptic ulcer bleeding, gastrointestinal tumors, gastric mucosal lesions, vascular dysplasia, esophagogastric variceal bleeding, and pancreatic and biliary tract injury, but also other anorectal diseases such as part colon and rectal cancer swelling or polyps, hemorrhoids, and anal fissure and other parts of bleeding such as epistaxis, thrombocytopenia, functional uterine bleeding, threatened abortion, and unexplained hematuria. Distal bleeding also involves syndromes of failing to keep part deficient and cold fluids in interior, such as nocturia, enuresis, clear nose, sweating, cold tears, and leucorrhea, and excessive gastrointestinal bleeding caused by anti-plate and anticoagulant drugs, unexplained positive in the fecal occult blood test, and other modern clinical new problems. The indications of Huangtu Decoction include not only lower blood, defecation before blood, distant blood, hematemesis, epistaxis, and other diseases in traditional Chinese medicine, but also three types of clinical manifestations including bleeding, deficiency syndrome, and stagnant heat syndrome. In the clinic, Huangtu Decoction can be used to treat acute upper gastrointestinal bleeding, acute coronary syndrome complicated with acute upper gastrointestinal bleeding, bleeding events caused by excessive antiplatelet and anticoagulant drugs, unexplained positive in the fecal occult blood test, gastrointestinal tumor with bleeding, thrombocytopenia, and other acute and critical diseases. The dosage of Cooking Stove Earthkey, Rehmanniae Radix, and Asini Corii Colla in Huangtu Decoction is the key to hemostasis.

ABSTRACT

Abstract Objective: To describe the use of nutritional supplements and blood status (hemogram, lipidogram, hepatic function, inflammatory markers, minerals, and homocysteine) in a sample of Brazilian T21 children with private health support before their first consultation with a T21 expert. Method: This descriptive cross-sectional study enrolled 102 participants. Brazilian families with a T21 member under 18 years old were contacted and those that consented answered a survey regarding socio-demographics and the use of nutritional supplements and shared the blood tests that their T21 members have collected for the first consultation with a T21 expert. Results: Frequencies and percentages were used to describe the variables. The most used supplements included vitamins (A, C and D), minerals (zinc and iron), omega-3, and antioxidants (curcumin). Hypothyroidism was observed in 56.9% of the participants. Hemogram alterations (increased hemoglobin, hematocrit, mean corpuscular volume, mean corpuscular hemoglobin and red cell distribution width, leukopenia, and lymphocytopenia), dyslipidemia, altered hepatic and inflammatory blood markers were frequently found. Conclusions: Nutritional supplements (mainly vitamins, minerals, omega-3 and antioxidants) are frequently used by Brazilian T21 children independently of professional counseling and/or supervision and should be a question to be raised during the clinical anamnesis since some of them may impact medical conduct. Moreover, many blood tests are altered in this population and clinicians should be aware of them in order to warrant an appropriate screening and the implementation of risk management measures as soon as possible and improve the general health of these persons.

ABSTRACT

【Objective】 To explore the population characteristics, the reasons for deferral and the changing trend of unqualified rate of blood test of voluntary blood donors in Xi′an, so as to provide a data support for recruiting low-risk regular blood donors, reducing unqualified rate, improving blood safety and blood quality. 【Methods】 The age, gender, occupation, education and unqualified blood test of voluntary blood donors in Xi′an area from 2012 to 2021 were classified and analyzed retrospectively. 【Results】 The number of voluntary blood donors in Xi′an increased year by year from 2012 to 2021, while the overall unqualified rate of blood test kept decreasing, with an overall unqualified rate at 2.36%, which was at a middle level in China. There were more male blood donors than female donors, and the highest proportion of blood donors in terms of age, occupation and education was between 21-30 years old group (40.19%), other occupation group(21.75%), and undergraduate and above group (32.61%) respectively. The total unqualified rate was ALT (0.68%)>HBsAg (0.55%) > anti -HCV (0.48%) > anti -TP (0.43%) > anti -HIV (0.17%). The unqualified rate of blood donors aged 31 to 40 was the highest. With the increase of academic qualifications, the unqualified rate generally showed a gradual downward trend. The unqualified rate of soldiers, teachers, civil servants and medical workers was low, while the unqualified rates of various items of students were high. There were seasonal differences in the total number of blood donors, the total unqualified rate and the unqualified rate of each item (except anti -HIV). 【Conclusion】 The voluntary blood donors are mainly males, aged 21 to 30, with other occupations and with bachelor degree or above. Regular voluntary blood donation teams in this area should be established among soldiers, teachers, civil servants, medical workers and highly educated groups, and more attention should be paid to health consultation for workers, farmers and students before blood donation.

ABSTRACT

【Objective】 To explore the status and characteristics of voluntary blood donors in rural areas of Dali Bai Autonomous Prefecture (referred as Dali), and to provide basis for scientific and effective voluntary blood donation in rural population in regions inhabited by ethnic groups. 【Methods】 The data of rural blood donors who donated blood in Dali from 2010 to 2019 were collected, including demographic data as nationality, gender, age, educational background, as well as the blood donation frequency and blood infection screening (index) results. The above data of urban blood donors who donated blood in Dali during the same period were selected to investigate the increasing trend of blood donation rate. SPSS26.0 was used for statistical analysis of the collected data of rural and urban blood donation population. 【Results】 From 2010 to 2019,the number of blood donors in Dali increased from 13 949 to 19 479,with an increasing rate of 39.64%. The number of rural blood donors increased from 2 623 to 8 727,among which the number of ethnic minority groups increased from 1 779 to 5 059.The ratio of male to female blood donors was 70.30% (1 844/2 623) vs 29.70% (779/2 623) in 2010,56.37% (4 919/8 727) vs 43.63%(3 808/8 727) in 2019. Those with educational level of junior middle school or below were the most, accounted for 43.97%(38 443/85 836),with ethnic donors of 24.47%(23 583/85 836). The proportion of donors aged between 36 and 45 was the highest[40.73% (30 477/74 827) ], with ethnic donors of [28.56% (21 374/74 827), and the proportion of repeated blood donors was 54.87%(35 279/64 299),with ethnic donors of 49.89%(18 080/36 240) [the proportion of repeated blood donors in urban donors in the same period was 48.13% (55 677/115 675) ] (P0.05). 【Conclusion】 It is of great significance to explore the characteristics of blood donors in rural areas (especially regions inhabited by ethnic groups) and the reasons for disqualification, in order to scientifically carry out the recruitment of voluntary blood donors and further promote blood donation for rural residents

ABSTRACT

The Tokushukai Group is one of the largest hospital groups in Japan. By connecting all the hospitals via a network, the group has been able to achieve centralized management of clinical information. The Tokushukai Group has been utilizing the data in various ways, such as participation in clinical trials, development of anticancer drug protocol systems, participation in BioBank Japan, and provision of data to MID‒NET®. These days, the Tokushukai Group has started to provide the data, named as the “Tokushukai Medical Database (TMD)”, to academia and industries for research purposes. The TMD mainly consists of inpatient and outpatient administrative claims data, Diagnosis Procedure Combination (DPC) data, electronic health records including blood test results and vital signs, and hospital-based cancer registry information. The strengths of the TMD include the availability of blood test results and vital signs in an standardized format, and the possibility to go back to each patient's electronic medical record from their claims data and DPC data, with the permission of headquarters and the ethics committee in the Tokushukai group. By taking advantage of these strengths, there have been increasing number of studies in the TMD, such as research on acute kidney injury defined as blood test results, hyperkalemia in dialysis patients, and validation study on rheumatoid arthritis. The issues newly raised during the utilization of data (e.g., echocardiography data not standardized) are expected to be resolved with the update of the network system for a wider range of research topics in the future.

ABSTRACT

【Objective】 To investigate the effect of AIDET communication mode in informing blood donors of reactive viral markers results. 【Methods】 Blood donors who came FOR COUNSELLING AFTER NOTIFICATION OF THEIR REACTIVE SCREENING TEST RESULTS from Jan 2018 to Sep 2021 were selected as the subjects. They were divided into two groups according to the blood donation time and notification mode: control group who were notified by a conventional mode(n=128), from Jan 2018 to Dec 2019, and the experimental group(n=92) who were notified by AIDET mode, from Jan 2020 to Sep 2021. The anxiety state and degree of satisfaction to the notification were compared between the two groups. 【Results】 After attending COUNSELLING, the anxiety level of the experimental group was significantly lower, while the degree of satisfaction to the notification was significantly higher than that of the control group (P<0.05). 【Conclusion】 AIDET communication mode established a standardized and effective communication channel and reduced the anxiety of blood donors WHEN NOTIFYING THE REACTIVE SCREENING TEST RESULTS. Moreover, it can enhance the trust and satisfaction between donors and medical staff as well as the quality of notification, which is of great significance to promote the healthy and continuous development of voluntary blood donation.

ABSTRACT

【Objective】 To explore the changes of blood routine, biochemical indexes and coagulation indexes of voluntary blood donors after COVID-19 vaccination, so as to provide a scientific basis for blood donation strategy and to ensure blood safety. 【Methods】 From March to August 2021, 55 blood donors who had received COVID-19 vaccination were sampled at the 2nd, 4th and 8th weeks after vaccination for blood routine, biochemical and coagulation indexes testing. The changes were analyzed. 【Results】 At the second week after vaccination, abnormal blood routine indicators occurred 16(5.82%) occasions, abnormal biochemical indicators 159(36.14%) occasions; at the 4th week after vaccination, abnormal blood routine indicators 14(5.10%) occasions and abnormal biochemical indexes 151(34.32%) occasions. There was no difference in blood routine and biochemical indexes before and after vaccination(P>0.05). Among the coagulation indexes, the incidence of abnormal increase of FⅧ activity at the 2nd, 4th and 8th weeks after vaccination were 34.5%(19/55), 40%(22/55) and 50.9%(28/55), respectively. The the incidence of abnormal increase of TT were 3.6%(2/55), 7.2%(4/55) and 21.8%(12/55), respectively. Both TT and FⅧ activity had a tendency of increasing(P<0.05), APTT had an decreasing trend(P<0.05), and FIB content took on dynamical changes(P<0.05). 【Conclusion】 The blood indicators of blood donors after receiving COVID-19 vaccination are almost within the normal reference values. They can donate regularly according to the required interval, which may help to improve the potential transient changes of coagulation function.

ABSTRACT

【Objective】 To analyze the changes of blood routine and platelet related parameters of the elderly apheresis platelet donors since the first donation, so as to ensure the health and platelet quality of the elderly apheresis donors. 【Methods】 From 2019 to 2021, a total of 69 apheresis platelet donors in our center, aged 55-60 years old, involving 3400 occasions of donation, were enrolled in this study. The change trends of blood routine and platelet related parameters before and after the age of 55 were retrospectively analyzed. One way ANOVA was used for group comparison, and Pearson correlation analysis of platelet parameters was performed. 【Results】 The values of blood routine and platelet related parameters were higher in men than in women (P0.05). As for the males in comparison of the parameters before 55 years old, WBC decreased significantly after 55 years of age (P0.05). The changes of PDW, MPV and P-LCR were consistent and showed a linear positive correlation. 【Conclusion】 The blood routine tests of the elderly apheresis platelet donors were all within the normal range and the proportion was relatively stable. Personalized blood donation schemes should be customized for the elderly blood donors and the health management of blood donors should be implemented.

ABSTRACT

Objective:To explore the relationship between blood test indicators and their changes and impaired fasting blood glucose, and to conduct risk prediction research.Methods:This study selected people who underwent a physical examination at a health management center in Jinan from 2018 to 2019, with an interval of 6 months or more between the two physical examinations, and had no history of diabetes and normal fasting blood sugar during the first year of the physical examination. We took the fasting blood glucose level in the second year as the dependent variable, and took the general examination and blood test indicators in the first year as well as the changes of these indicators as independent variables to construct the model. First, the random forest model was used to screen the independent variables, and then the logistic regression model is fitted to calculate the effect values of the independent variables.Results:A total of 4 416 subjects were enrolled in this study. After a 1-year follow-up, 3.45% (133/3 851) subjects developed prediabetic blood glucose, and 0.42% (16/3 851) developed diabetic blood glucose. The sensitivity of the model using blood test indicators to predict the risk of impaired fasting blood glucose after 1 year was 67.1%, and the specificity was 100.0%. The risk of impaired fasting blood glucose after 1 year was high in the population with older age ( OR value was 1.024, 95% CI 1.014-1.035), higher systolic blood pressure ( OR value was 1.018, 95% CI 1.011-1.025), higher body mass index ( OR value was 1.079, 95% CI 1.036-1.125), higher uric acid ( OR value was 1.003, 95% CI 1.002-1.005), higher glutamyl transpeptidase ( OR value was 1.006, 95% CI 1.003-1.010), lower creatinine ( OR value was 0.971, 95% CI 0.957-0.985), lower AST/ALT ( OR value was 0.614, 95% CI 0.411-0.917)) and lower estimated glomerular filtration rate ( OR value was 0.972, 95% CI 0.958-0.988). The sensitivity of the model using the changes of blood test indicators to predict the risk of impaired fasting blood glucose after 1 year was 65.2%, and the specificity was 99.7%. The risk of impaired fasting blood glucose is high in the population with increased triglycerides ( OR value was 1.152, 95% CI 1.057-1.257), increased alkaline phosphatase ( OR value was 1.021, 95% CI 1.012-1.030), increased albumin ( OR value was 1.087, 95% CI 1.029-1.148), increased aspartate aminotransferase ( OR value was 1.006, 95% CI 1.003-1.009), increased weight ( OR value was 1.049, 95% CI 1.019-1.081) within 1 year. Conclusions:The possibility of impaired fasting blood glucose after 1 year is high in the population with older age, higher systolic blood pressure, higher body mass index, higher uric acid, higher glutamyl transpeptidase, lower creatinine, lower AST/ALT and lower estimated glomerular filtration rate. The possibility of impaired fasting blood glucose is high in the population with increased triglycerides, increased alkaline phosphatase, increased albumin, increased aspartate aminotransferase and increased weight with in 1 year. When the above situation occurs in the physical examination, the blood glucose should be monitored and timely intervention should be taken to prevent the development of diabetes.

ABSTRACT

There are four methods for fecal detection of colorectal cancer (CRC) markers: fecal occult blood test, fecal DNA test, fecal microRNA test, and fecal fusobacterium nucleatum (Fn) test. Fecal immunochemical test has been recommended by experts at home and abroad as the first choice for CRC screening. Fecal DNA test, due to its high price, has not yet been screened for large samples of people in China, so it is recommended as the second level of CRC screening. Fecal microRNA detection has been paid more and more attention by researchers. In recent years, the detection of fecal microbial markers has become more and more popular, especially fecal Fn detection, which is expected to become a microbial indicator for CRC screening.

【Objective】 To analyze the difference of the internal control (IC) Ct value of two sets of blood nucleic acid detection(NAT) systems for individual donation and explore the feasibility of the IC Ct value used as stability evaluation index of NAT system. 【Methods】 A total of 4342 blood samples were randomly selected from our center from June to November 2020, and the IC Ct values of NAT system were classified into three categories: 1) classified by detection items: HBV, HCV and HIV; 2) classified by sample type: negative control (NC), positive control (PC), internal quality control (QC), blood donor sample (S); 3) classified by different instruments: system A and system B, and the differences of IC Ct value were analyzed and compared. 【Results】 A total of 14 196 IC Ct values of NAT were collected, showing a non-normal distribution. 1) classified by detection items: there were differences in the median of overall IC Ct values of HBV, HCV and HIV (H=859.6, P0.05), except for HBV QC (Z=1 635)and S (Z=1 848 560), HCV S (Z=1 957 844), HIV S (Z= 2 217 855) (P<0.01). No difference in IC values (P<0.01) was noted for any detection items (NC, PC, QC and S) in the same set of detection system. 【Conclusion】 Although IC values trended to be different due to types of sample, the NC, PC, and QC samples that come with the original reagents were identical in the two detection systems. The change of IC Ct value may be used as an evaluation index for the stability of the detection system.

【Objective】 To investigate the age, gender and unqualified blood test of voluntary blood donors, so as to provide a data support for blood donation publicity, recruitment and retainment of blood donors, and to avoid unnecessary waste of blood resources. 【Methods】 The age, gender and unqualified blood test of voluntary blood donors in Lianyungang area from 2017 to 2020 were retrospectively classified and statistically analyzed. 【Results】 The proportion of voluntary blood donors increased with age, and the proportion of blood donation in the 51-60 age group decreased. The unqualified rate of voluntary blood donors showed a downward trend with the increase of age. The unqualified rate of blood test in 51-60 age group was rather low. Except for 18-20 age group, the blood donation ratio and the unqualified rate of blood test of male blood donors in all age groups were higher than those of female blood donors, which were 1.47∶1 and 1.30∶1, respectively. The total unqualified rate of blood test was 1.09%(1 274/117 079), and it decreased year by year. The unqualified item of blood test was ALT>HBV>TP>HCV>HIV, and the unqualified rate of ALT test between male and female blood donors was significantly different(P<0. 05), and the unqualified rate of HBV and TP test in different time periods was significantly different(P<0.05). 【Conclusion】 The publicity of voluntary blood donation for people aged 18-25 and 50-60 should be strengthened, and regular blood donor team should be continuously retained and expanded. Effective consultation and preliminary blood screening before blood donation are of great significance in reducing the unqualified rate of blood test. The unqualified rate of HBV and TP tests can be reduced by scientific preliminary screening process and standardized operation, which is of great significance to reduce the waste of blood resources and ensure blood safety.

ABSTRACT

The blood donation, component preparation and manufacturing, laboratory, issuing and quality control were studied and compared between the UK and China, in order to learn British experience in the clinical practice and blood services. The age limits of blood donors, blood collection units, donation times per year, laboratory items, and the types(volumes) of component preparation and manufacturing in the UK are more superior than those in China. In addition, the blood quality monitoring and regarding indicators are more scientific and reasonable in the UK. The full reimbursement of clinical blood expenses for patients has been realized in the UK. The British experience in continuous safeguard of the blood safety and balance of requirement and availability contributes to the constant and scientific development of British blood services over the years, and is worthy of references.

ABSTRACT

Introducción: El cáncer colorrectal (CCR) es la segunda causa de muerte por cáncer en Argentina. Debido a su alta prevalencia es fundamental normatizar un programa de pesquisa para la prevención y detección temprana. La precisión del test de sangre oculta en materia fecal inmunológico (SOMFi) para pesquisa de CCR en población de riesgo promedio ha demostrado ser adecuada según la bibliografía internacional, no habiendo, sin embargo, información a nivel local. El objetivo es evaluar en nuestro medio la precisión diagnóstica del test de SOMFi en una única ronda para la pesquisa de CCR en pacientes de riesgo promedio. Diseño: Prospectivo de precisión diagnóstica. Material y Método: Se incluyeron pacientes con riesgo promedio que consultaron para realizar una videocolonoscopía (VCC) por pesquisa de CCR en el Hospital Alemán de Buenos Aires, entre el 1 de junio del 2015 y 31 diciembre de 2017. Se excluyeron todos los pacientes con riesgo incrementado para CCR. Todos los pacientes realizaron el test de SOMFi y posteriormente la VCC. Los endoscopistas estaban ciegos para el resultado del test al momento de realizar la VCC. Se evaluó la precisión diagnóstica del test SOMFi para detectar lesiones neoplásicas avanzadas (LNA) calculando la sensibilidad (S), especificidad (E), valor predictivo positivo (VVP) y negativo (VVN), coeficiente de probabilidad positivo (CP+) y negativo (CP-). Se evaluó también la precisión para la detección de adenomas de bajo riesgo, pólipos aserrados y CCR. Resultados: Se incluyeron un total de 300 pacientes; 273 (91%) entregaron la muestra de materia fecal para realizar el test de SOMFi y completaron la VCC. La edad media de los pacientes fue de 56.9 (40-85) años y 54% fueron hombres. Del total de pacientes que realizaron ambos estudios (273), 53 pacientes (19%) presentaron al menos un adenoma de bajo riesgo, en 18 pacientes (6,59%) observamos al menos un adenoma aserrado sésil y en 21 pacientes (7,7%) al menos una lesión neoplásica avanzada (LNA). Solo 4 pacientes (1.5%) presentaron CCR. En cuanto a la precisión diagnóstica del test de SOMFi en una única ronda para detectar LNA observamos una S de 30%, E de 84%, VPP de 13% y un VPN de 94%. Para adenomas de bajo riesgo observamos una S de 13%, E de 84%, VPP de 17%, VPN de 79%. Para adenomas aserrados sésiles observamos una S de 16.7%, E de 87%, VPP de 11% y de VPN 91%. La precisión para el CCR fue la siguiente, S de 75%, E de 83%, VPP 6%, VPN 99%. No se observaron complicaciones post procedimientos. Conclusiones: La precisión diagnóstica del test de SOMFi en nuestro medio es comparable a los resultados internacionales. Sin embargo, la baja precisión observada en una única ronda realza la necesidad de realizarlo de forma anual o bianual para poder optimizar su precisión y lograr programas de pesquisa efectivos.(AU)


Background: Colorectal cancer (CRC) is the second leading cause of cancer death in Argentina. Due to its high prevalence, it is essential to standardize a screening program for prevention and early detection. According to international literature, the accuracy of the immunochemical fecal occult blood test (FIT) for CRC screening in an average-risk population has proven to be adequate, but there is no information at the local level. Objective: To evaluate the diagnostic accuracy of the FIT test in a single round for CRC screening in average-risk patients in our setting. Design: Diagnostic accuracy prospective study. Material and Methods: Average-risk patients who consulted for a CRC screening video colonoscopy (VCC) at the Hospital Alemán of Buenos Aires, between June 1, 2015 and December 31, 2017 were included. All patients with increased risk for CRC were excluded. All patients performed FIT and subsequently VCC. The endoscopists were blind to FIT result at the time of VCC. The diagnostic accuracy of FIT to detect advanced neoplastic lesions (ANL) was evaluated by calculating sensitivity (S), specificity (Sp), positive predictive value (PPV), negative (NPV), positive likelihood ratio (LR+) and negative likelihood ratio (LR-). The accuracy for the detection of low-risk adenomas, serrated polyps and CRC was also evaluated. Results: A total of 300 patients were included; 273 (91%) submitted the stool sample to perform FIT and completed VCC. The mean age of patients was 56.9 (40-85) years and 54% were men. Of the total number of patients who carried out both studies (273), 53 (19%) patients had at least one low-risk adenoma, 18 (6.59%) patients had at least one sessile serrated adenoma and 21 (7.7%) patients had at least one ANL. Only 4 (1.5%) patients presented CRC. The diagnostic accuracy of FIT in a single round to detect ANL was: S 30%, Sp 84%, PPV 13%, NPV 94%; for low-risk adenomas: S 13%, Sp 84%, PPV 17%, NPV 79%; for sessile serrated adenomas: S 16.7%, Sp 87%, PPV 11%, NPV 91% and for CRC: S 75%, Sp 83%, PPV 6%, NPV 99%. No post-procedure complications were observed. Conclusions: The diagnostic accuracy of FIT in our setting is comparable to international results. However, the low precision observed in a single round highlights the need to do it annually or biannually in order to optimize its accuracy and achieve effective screening programs. (AU)

ABSTRACT

Objective To evaluate the association between blood test parameters and intensity of Plasmodium falciparum infections among imported falciparum malaria cases in Tianjin City from 2015 to 2019, so as to provide insights into the early diagnosis of imported P. falciparum malaria. Methods The epidemiological data of 37 imported cases with confirmed diagnosis of P. falciparum malaria in Tianjin City from 2015 to 2019 were collected, and the epidemiological features and clinical manifestations were retrospectively analyzed. In addition, the association between blood test parameters and intensity of P. falciparum infections was evaluated among the imported P. falciparum malaria cases. Results Among the 31 imported P. falciparum malaria cases, there were 31 cases (83.8%) with a reduction in platelet (PLT) counts, 16 cases (43.2%) with a reduction in red blood cell (RBC) counts, 16 cases (43.2%) with a reduction in hemoglobin (Hb) concentrations, 23 cases (62.2%) with a rise in neutrophil percentage (NEUT%), 32 cases (86.5%) with a rise in total bilirubin (TBIL) concentrations, 29 cases (78.4%) with a rise in alanine aminotransferase (ALT) concentrations, 28 cases (75.7%) with a rise in aspartate transaminase (AST) concentrations, and 23 cases (62.2%) with a rise in gamma-glutamyl transpetidase (GGT) concentrations. The PLT count and Hb concentration correlated negatively with the intensity of P. falciparum infections (Goodman-Kruskal γ = –0.568 and –0.521, both P values < 0.05) and the TBIL concentration and NEUT% correlated positively with the intensity of P. falciparum infections (Goodman-Kruskal γ = 0.496 and 0.610, both P values < 0.05) among imported falciparum malaria cases; however, there were no associations of ALT, AST, GGT levels or RBC count with the intensity of P. falciparum infections among the imported falciparum malaria cases (Goodman-Kruskal γ = 0.370, 0.497, 0.314 and –0.434, all P values > 0.05). Conclusions PLT, Hb, TBIL and NEUT% may serve as markers for early auxiliary diagnosis of imported P. falciparum malaria, and PLT and TBIL may provide valuable information for the diagnosis of severe imported P. falciparum malaria.

Introduction@#According to CDC guideline, common changes in patient admitted due to pneumonia caused by COVID-19 are lymphopenia (63%), leukocytosis (24-30%), leukopenia (19-25%). Neutrophil-lymphocyte ratio (NLR), lymphocyte-monocyte ratio (LMR), thrombocyte-lymphocyte ratio (PLR) are important to evaluate prognosis of infectious diseases as well as cancer. </br> Hematological tests are important for diagnosis, treatment and monitoring of patient with COVID-19. Our study objective was to determine the changes of leukocyte, lymphocyte, thrombocyte, NLR, LMR, and PLR in imported cases of COVID-19 into Mongolia.@*Method@#The data such as age, gender, and laboratory test of a total of 249 cases who admitted to National Center forCommuncable Diseases (NCCD) from March 11 to July 20, 2020 was collected from information system of clinical laboratory of NCCD. Peripheral blood tests were conducted by XN 550 which is fully automated hematological analizator of Sysmex corporation, Japan. Statistical analysis such as mean, standard deviation, probability range was done by Microsoft Excel SPSS -25 program.@*Result@#Regarding to gender, 163 (66.5%) were male and 86 (34.5%) were female. Mean age was 26 with range age of 11-80. Statistic analysis on leukocyte (mean 6.38 x 10<sup>9</sup>/L; P <0.000), thrombocyte (mean 283 x 10<sup>9</sup>/L P <0.000), neutrophil (mean 3.33 x 10<sup>9</sup>L; P <0.000), lymphocyte {mean 2.3 x 10<sup>9</sup>/L; P <0.000), NLR (mean 1.6, P <0.000), PLR parameter (mean 141.8 P <0.000> were revealed. Leukopenia (<3.98) were in 17 (6.8%), leukocytosis (>10.0) were in 11 (4.4%) cases. Lymphopenia (<1.18) were in 14 (5.6%), lymphocytosis (>3.74) were in 14 (5.6%) cases. Thrombocytosis (>369) were in 21 (8,4%), thrombocytopenia (<163) were in 4(1.6%) cases. Neutropenia (<1.56) were in 14 (5.6%) neutrophilia (>6.13) were in 15 (6%). The rest 220 (88.3%) cases have been determined no changes. PLR were 141.8 ±88.6, NLR were in 1.29 ±1.26 in 249 cases. Increased NLR and decreased PLR were in 10 (4.0%) and 114 (45.7%); increased PLR and decreased PLR in 31 (12.4%) and 28 (11.2%), respectively.@*Discussion@#Our result which is leukocytosis in 4.4%, leukopenia in 6.8%, lymphopenia in 5.6% of all imported eases ol"CO\ 1D-W. are similar to other studies. However, percentage of changes were lower than similar studies due to low rate of severe cases. It suggests, further studies clinical stages and severity of the infection need to be conducted.

Resumo Foram avaliados os efeitos do destreinamento sobre parâmetros cardiovasculares em idosas. Idosas foram submetidas a oito semanas de treinamento resistido seguidas de oito semanas de destreinamento. Foram avaliadas nove mulheres idosas (idade 62 ± 2,30). Foram feitas coletas de sangue venoso periférico e avaliação da composição corporal antes do início de treinamento, após o treinamento e após a fase de destreinamento. Não houve aumento significativo da PCR na fase de destreino, porém observamos alterações negativas para colesterol total e composição corporal, representada pelo peso gordo. Os resultados sugerem que oito semanas de destreinamento não aumentaram significativamente a PCR, porém influenciaram negativamente em outros parâmetros relacionados aos riscos cardiovasculares, como valores antropométricos e bioquímicos representados pelo colesterol total e massa gorda, respectivamente. De fato, a continuidade do treinamento físico é essencial para adquirir e manter uma boa saúde, caso contrário os benefícios alcançados regridem aos valores iniciais.


Abstract The detraining effects on cardiovascular parameters in the elderly women (EW) were evaluated. EW underwent 8 weeks of resistance training, followed by eight weeks of detraining. 9 EW were evaluated (age 62 ± 2.30). Peripheral venous blood collections and body composition evaluation were performed before training, after training and after the detraining phase. There was no significant increase in CRP in detraining phase, but we observed negative changes for total cholesterol and body composition, represented by fat weight. The results suggest that 8 weeks of detraining did not significantly increase CRP, but had a negative influence on other parameters related to cardiovascular risks, as anthropometric and biochemical values represented by total cholesterol and fat mass, respectively. In fact, continuity of physical training is essential to acquiring and maintaining good health, otherwise the beneficial adaptations achieved will return to initial values.


Resumen Se evaluaron los efectos de la falta de entrenamiento sobre los parámetros cardiovasculares de mujeres mayores. Estas siguieron un entrenamiento resistido durante 8 semanas, seguidas de 8 semanas de falta de entrenamiento. Se evaluó a 9 mujeres de edad avanzada (edad de 62 ± 2,30). Se realizaron extracciones de sangre venosa periférica y evaluación de la composición corporal antes del inicio del entrenamiento, después del entrenamiento y después de la fase de falta de entrenamiento. No hubo un aumento considerable de la PCR en la fase de falta de entrenamiento, pero observamos alteraciones negativas del colesterol total y composición corporal, representada por la grasa. Los resultados sugieren que 8 semanas de falta de entrenamiento no aumentaron considerablemente la PCR, pero influyeron negativamente en otros parámetros relacionados con los riesgos cardiovasculares, como valores antropométricos y bioquímicos representados por el colesterol total y la masa grasa, respectivamente. De hecho, la continuidad del entrenamiento físico es esencial para adquirir y mantener una buena salud; de lo contrario, las adaptaciones beneficiosas alcanzadas vuelven a los valores iniciales.

Owing to its high resistance to corrosion in a physiological environment and the excellent biocompatibility that gives it a passive, stable oxide film, titanium is considered the material of choice for intraosseous use. There are certain studies which show titanium as an allergen but the resources to diagnose titanium sensitivity are very limited. The allergic response to titanium is reported in very few cases. The purpose of this article is to create awareness among the people handling and using titanium in dentistry so that better strategies can be developed to manage the allergic response to it

ABSTRACT Globally, colorectal cancer is the third leading cause of cancer death among men and the second among women, corresponding to about 10% of all cancers. The Brazilian Ministry of Health and National Cancer Institute recommend the screening of colorectal cancer for people over 50 years-old with Fecal Occult Blood Test. Endoscopy is limited to patients with positive screening results. The aim of this study is to evaluate the incidence of malignant or premalignant lesions diagnosed by endoscopy in patients with positive or negative Fecal Occult Blood Test and assess the efficacy of Fecal Occult Blood Test to predict the finding of a malignant colorectal lesion. We carried out a cross-sectional study among patients with a Fecal Occult Blood Test result that were submitted to colonoscopy, in the same hospital, from March 2016 to July 2017. Sensitivity, specificity, positive and negative predictive value of Fecal Occult Blood Test compared to colonoscopy neoplastic findings was calculated. The total of 92 patients were enrolled, 52 (56.5%) were female, Fecal Occult Blood Test was positive in 42.4% of them and in 41 (44.6%) the colonoscopy showed abnormal findings. Polyps were the most frequent alteration, found in 20 patients (21.7%). Among the patients with polyps, 15 (16.3%) had neoplastic and 5 (5.4%) presented non-neoplastic polyps. The Fecal Occult Blood Test sensitivity for detection of neoplastic polyps was 66.7%, specificity 62.3%, positive predictive value 11% and negative predictive value was 94.2%. Considering the need for a screening method, Fecal Occult Blood Test showed to be an effective and reliable screening test that can be applied in public health programs to detect and prevent colorectal cancer.


RESUMO Globalmente, o carcinoma colorretal é a terceira principal causa de morte por neoplasia entre homens e a segunda entre mulheres, correspondendo a 10% de todas as neoplasias. O Ministério da Saúde Brasileiro e o Instituto Nacional do Câncer recomendam a triagem do câncer colorretal para indivíduos acima de 50 anos, utilizando a Pesquisa de Sangue Oculto nas fezes. A endoscopia é reservada para aqueles com Pesquisa de Sangue Oculto nas fezes positiva. O objetivo deste estudo é avaliar a incidência de lesões malignas/pré-malignas diagnósticas na colonoscopia e correlacionar com os resultados prévios da Pesquisa de Sangue Oculto e verificar a eficácia da Pesquisa de Sangue Oculto para predizer uma lesão colorretal maligna. Realizamos um estudo transversal em pacientes que apresentavam resultados positivos ou negativos de Pesquisa de Sangue Oculto nas fezes e foram submetidos à colonoscopia, na mesma instituição, entre março de 2016 e julho de 2017. Dos 92 participantes; 52 (56,5%) eram do sexo feminino, a Pesquisa de Sangue Oculto nas fezes foi positiva em 42,4%; e em 41 (44,6%) a colonoscopia mostrou alterações. Em 20 pacientes (21,7%) havia pólipos; 15 (16,3%) eram neoplásicos e 5 (5,4%) não neoplásicos. A sensibilidade da Pesquisa de Sangue Oculto nas fezes para detecção de pólipos neoplásicos foi 66,7%; a especificidade 62,3%; o valor preditivo positivo 11% e o valor preditivo negativo 94,2%. Considerando a necessidade de um método de triagem, a Pesquisa de Sangue Oculto nas fezes mostrou ser um exame de triagem eficaz e confiável para ser aplicado em programas de saúde pública com o objetivo de detectar e prevenir o carcinoma colorretal.
